The 'beauty and the beast' of night shift
Tonight is my last night shift in gartnavel for this year. It has been quite good, i have to admit. Totally different from night shift in western. Most of the nights have been quiet except for last thurday. It was quite exciting though...every night there s usually one patient becomes hypotensive, one tacypnoeic and one tachycardic. Couple of few more becomes pyrexial, which usually happened bout 7 am in the morning after the obs round. That night, one lady in ENT ward started to bleed to her nose again, well, she came in with epistaxis, get her nose pack and cauterised, but still she was bleeding through the pack and it was soaking all through. Our usual reflex is when somene is bleeding u give lots of fluid, usually stat as that person most of the time will start to become hypotensive, but it was different this time. The BP came back as high instead, it was bout 164/85...at that time, i was panic, seriously dunno what to do...she got fluid going through anyway, but her high BP makes me looks really stupid as i was blank, no idea of what am i supposed to do...and yet, she is still bleeding.The incidence was a good experience...we manage to get the Spr on call to come in and cauterised the vessels which were bleeding. And she actually loss bout 5g of Hb due to the epistaxis...I was surprised how much bloods we can lost just by bleeding through the nose.
Anyway, I do enjoyed my night shifts as usually i have 2 hours rest doing nothing , maybe because there s not many patients on the ward as christmas coming, so those on the ward are the one that have to stay in the hospital. In fact, last night I manage to sleep for two hours, only that they dun provide on call doctors with a mess or at least bed for us. So i had to sleep on the chairs in the doctor's room, which happen to be quite comfy..haha
As compared to western shift, it was really busy, sometime u dun even have time to eat...it s just continuous the whole night...with admission in a&e and out of hours gp...
